Transition
Transitions can be challenging for our students and therefore they need to be carefully managed. Whether this is their first experience of school, moving to another class or their next steps away from Pathfield. It is vital that any transition plan enlists the views of everyone including staff, families and especially has the child at the centre.
At Pathfield School, transition processes are tailored to the individual as we recognise needs can be very different. During any transition period, it is important that children can communicate their concerns and are supported to cope with any re-adjustments. Some children may need more support than others, therefore starting early with talks about transition can be very beneficial for them.
Transition Event
There is an annual transition event held at the school for parents/carers and young children from Year 9 and above. Here you can find out about local services, Pathfield 6th Form and other FE establishments, supported living, respite facilities and financial advice for the future. Destination Data
Destination data is any information that is collected on where a student goes and what they do after leaving school. At Pathfield School we track students for 3 years after leaving school in KS4 or the 6th Form. This helps to identify those young people that are not in education, employment or training (NEET) and to offer them support. We understand our duty to inform the Local Authority whenever a pupil under the age of 18 leaves education before completion. A summary of the last three years can be seen below:

Every SEND student and their parents/carers should have access to good quality information about future study options, labour market opportunities and the Local Offer.
